Office hours are by appointment and include evenings, weekends, internet and phone sessions.
Monday-Thursday 8:00 AM to 7:00 PM
Friday-8:00 AM-2:00 PM
Saturday- By Appointment Only
Sunday Closed
Provider for the following plans depending on what your plan is: Aetna, Cigna, Cigna Eap, Humana, Medicare, and Sliding Fee Scales That are Affordable and Reasonable.
Ms. Levine does not take on any court cases or case that are involved with the legal system, Ms.Levine, is a mental health professional and provides therapeutic services only.
Sessions are 45 minutes in length
Co-Pays Due Upon Appointment Time! if you are unaware of what your co pay is, a charge will be applied at the time of session and will be placed towards your co pay or deductible.
Session Fees are $150.00
Initial Appointment is 130.00 if you have insurance we will bill after first appointment
Cancellations must be made within 96 hours or there is a $85.00 fee.
Out of Network Provider for Different Insurance Policies Just Ask! if you cancel right before the session or the day of, you will be billed 160.00, this is non negotiable. Cancellations must be prior to 96 hours.
All consents must be filled out before session by all legal guardians and paper work must be filled out before session.
All potential patients must have a 15 minute interview prior to making an appointment.
Fees are on a sliding fee scale and will be assessed and determined on a case by case situation. I offer special rates for non insured clients. I want to ensure that people who need help get the help they need.
Therapy sessions are held in office, virtual or a place where you are comfortable. Sessions are also held via phone, or internet. Being relaxed is one way to open the lines of communication with a therapist, so you can discuss whatever issues you are dealing with.
Please when you contact us, provide a copy of insurance card, address and date of birth.
INSURANCE FORM MUST BE FILLED OUT PRIOR TO AN APPOINTMENT
All Co-Pays and Payments are due upon arrival and all payments must be made at the time of the appointment. Sessions will not be held until payment is made
